Make sure upgrade uses the same schema as installation.

- Removed vendor info in fields from schema.xml
- Mapped VARLONGCHAR to mediumtext
- Database upgrades now uses field attributes instead of vendor info.
- Updated app_cache_view files to conform to schema.sql
